Initial Nutritional Therapy Consultation

Must Allow A Minimum of 4 Days Before Booking To Allow For Online Forms To Be Completed And Evaluated Your Initial Consultation includes: Nutritional Assessment Questionnaire (NAQ) – This is done prior to your appointment online and provides me with valuable information regarding your symptoms. Health History Background – A thorough health history is taken during your initial consultation in order to put together the pieces of what is going on in your body. You will complete an online Health History prior to your session. Functional Evaluation & Testing – Based on your health history and NAQ results testing will be done to gather more information as to what is going on in YOUR body. Areas that may be addressed include the following: Digestive Health, Food Sensitivities, Yeast Overgrowth, Hormonal Imbalances, Blood Sugar Stability, Brain Chemistry Imbalances, and/or Toxin Overload. Dietary Analysis – You will be required to keep a three-day food journal prior to your visit. Your current diet will help to create a program for you that will be easy to transition into. Forms will be emailed to you at least a week prior to your appointment. Personalized Nutrition Plan – Your personalized plan will be discussed at the end of your initial appointment. Your detailed Personalized Nutrition Plan will then be emailed to you based on the information obtained in the consultation as well as your primary goals. A short and long term program will be created in order to optimize success in reaching your goals.
